Required Documentation

Documentation regarding the disability should be provided to the Disability Services Coordinator. The cost of obtaining this documentation is the student’s responsibility. Additional information may be required if the initial documentation is incomplete or inadequate to determine the extent of the disability and reasonable accommodations. This is done at the discretion of the Disability Services Office. The Disability Services Office reserves the right to deny services or accommodations pending receipt of documentation. Appropriate documentation must meet the following criteria:

Hearing Impaired - Documentation must be from a licensed audiologist and must include a diagnosis and how the disability impacts the student in an academic environment.

Learning Disabled - Documentation must be from one of the following types of licensed psychologists: clinical, educational, school, neuropsychologist, or from a learning disability specialist. Documentation must include a diagnosis and list of reasonable accommodations.

ADD/ADHD - Documentation should be from a qualified professional (e.g., physician, licensed psychologist or neuropsychologist) and must include a diagnosis and list of reasonable accommodations.

Physical Impairment - Documentation must be from a licensed medical doctor that includes diagnosis and how the disability impacts the student in an academic environment. The physician cannot be related to the student.

Psychological Impairment - Documentation must be from a licensed psychologist, psychiatrist, or medical doctor that includes diagnosis and how the disability impacts the student in an academic environment.
